Vaccine Information


On Nov. 4, Memorial announced planned updates to the Procedure for Vaccine Requirement to align with the provincial government’s Mandatory Vaccination Policy. Memorial’s updated procedure is now posted.It reflects the requirement that all students, staff, faculty, vendors and contractors at Memorial University be fully vaccinated against COVID-19 or have an approved accommodation. Rapid or PCR testing will not be an option to opt out of vaccination under this revision. The Nov. 4 announcement detailing these revisions can be found here.

Students, faculty and staff will require the first dose of an approved COVID-19 vaccination by Sept. 7, 2021, and the second dose by Oct. 15, 2021, unless granted an exemption. Recognized vaccines include those approved by the World Health Organization (WHO) and/or Health Canada.

Memorial has a secure vaccine declaration form. You can access this form directly in the My MUN portal. If you have any technical issues accessing the form, please contact the help desk. Memorial values the privacy of your information and the form is the most secure way to declare your vaccine status.

Please complete this form at your earliest convenience and no later than the end-of-day Sept. 7. This form integrates the process for exemptions to the vaccine mandate. If you submit by Sept. 6, you can choose to enter a prize draw for one of 20 prizes.

Residents of Newfoundland and Labrador who have received the COVID-19 vaccine can access their vaccination record online. To get vaccinated, please find on-campus vaccine clinic information or regional health clinic information on our website.
